Kentucky Report Talks About the Dangers of Flying Debris

Incident report produced by Kentucky’s State FACE program details an incident involving a press operator being struck by a metal fragment and steps that can be taken to mitigate these dangers.

A case report developed by the Kentucky Fatality Assessment and Control Evaluation (FACE) Program outlines the dangers posed by flying metal debris in the workplace, specifically fragments that could be ejected while using a hydraulic press. The report cites a January 2023 incident that resulted in the death of a worker from Kentucky and outlines steps that can be taken to mitigate future incidents.

Kentucky FACE is a National Institute for Occupational Safety and Health (NIOSH) funded occupational fatality surveillance program. Its goal is to prevent fatal work injuries by studying the worker, the work environment, and the role of management, engineering, and behavioral changes in preventing future injuries.
